Follow the steps below to schedule an appointment. Once your appointment has been scheduled, you will receive instructions on how to register for the Patient Portal. You will also receive intake paperwork to be completed before your appointment. If you have any questions or need assistance, please give our care team a phone call at (888) 341-6401.

Step 2: Complete all intake paperwork prior to your appointment
Once your appointment is scheduled, you'll receive all your intake paperwork online for you to complete quick and easy.
Step 3: Attend your appointment and meet with a licensed medical provider
Whether your first visit is an in-person or a virtual one, we'll be there ready to meet with you and provide you high-quality care!
Step 4: Begin receiving treatment as a Priority Patient!
During and immediately after your visit, any prescriptions will be sent to your pharmacy of choice and your care coordinator will follow-up to check-in periodically.

Our telehealth and virtual visits are conducted using a secure, HIPAA-compliant platform, ensuring your privacy and safety. This ensures that when an in-person visit is essential, especially for patients with specific conditions or treatment plans (like those on controlled substances), you can receive care closer to home and within your community.
